    What are reasons for changing a child's last name in New York
    What are acceptable reasons for changing my sons last name to mine in New York. His donor is willing to sign the consent. But he has had no contact with him for over a year and I had to track him down through Facebook to get ahold of him. Is abandonment a reason for his name change??

    You don't need a reason - you file the application and supporting documents. It's just that easy.

    If you had a donor then just bring the papers with you from the clinic that worked with you and go through the courts to set the record straight.

    If the bio father (not donor) listed on the birth certificate will agree to the name change then just petition the court for the change.
