    Is there any precidence for asking the court to add my maiden name (once restored) to my children's surname? What is the most effective way to go about that if their father has refused to give consent to the addition?

    To do a legal name change requires a court order. To change a minor's name generally requires the approval of both parents.

    It would depend on the law of the particular state in which you and the child live.
