I seem to remember those were related to sports or other athletic activities...but its been a lot of years.

No, I remember my sons were required (by the state) to get checkups and shots when entering school, at maybe 3rd or 4th grade and again at 7th or maybe when entering high school. When changing schools it was a must to be up-to-date with everything.

It probably differs by state.

In NY there's a school nurse annual physical and a check for required innoculations.

When I was in grade school dinosaur fever was a problem, so we all got shots.
