    Forms to relinguish parental rights/GA
My husband wants to adopt my son who has never known his biological father. The biological father hasn't been apart of the child's life for the past 6 years. I've been married for 5 years. We are going to court for the adoption. However, I would like to find the form to give up parental rights before... Can anyone help... Please

There is no fill in the blank form in GA, next you should not be trying an adoption without an attorney.

The motion to give up rights for an adoption is part of the entire adoption filing. Unlike some states GA does not make it easy with blank forms.

But the bio father will have to sign them to give up his rights and this included with the filing.
