Hot water pressure regulator

I have an issue with hot water pressure in the bathroom. I turn the shower hot water on only get a bit more than a trickle come out. I have to turn on the bathroom basin hot water fully on and fully off than I get a full shower pressure. I was thinking it could be the pressure regulator. Any ideas?

I have a pressure regulator for the water coming into the home. However, when I read the pressure with a gauge it still shows as 135 psi. When water is flowing in the house, the pressure reads 65 psi and is adjustable. I can adjust is down really far to 15 psi and then adjust back up the 65 psi. ...
