Eviction of a partner, not married. Lived together for 4 years
Living together as a couple, I want her to leave, she refuses to believe me. I want to be kind, but I know with her I must follow through. She has known that I want her to move out, even offered to help her get a place. All she does is scream at me. After three years I have had enough. I just need forms, so when we do the Intervention I want her to see I am serious. And hopefully I won't have to file them.
I need to know what are the proper forms for a roommate eviction, and for a order of protection. I want to cover all contingencys. Thanks

    Landlord/tenant laws vary by state.
And are you the owner or the tenant? Does she have a lease? Do you if you are renting?
You give notice in writing, file eviction papers, and follow the laws of your locale, regardless of whether she ever paid a dime or has no contract.
You get a RO if she gets violent or damages the place, not as a standard part of an eviction. The final step in an eviction is having a sheriff or other authority remove her.

As noted we can't help specifically without knowing your general locale.

You can only get her out if she is neither a leaseholder or part owner. If that is the case, then you give her a written notice that simply says:

Please be informed that your residency at <insert address> is terminated as of <insert date 30 days in the future>. Please vacate the premises on or before that date.

If she doesn't leave, then you will need to go to court for an eviction order. If she gets violent call the police to remove her and then ask for a restraining order.
