    Remove my name from birth certificate
    Never married, I was with my girlfriend at the time she became pregnant. Never thought it wasn't mine tell a month after our girl was born. I went to a lab and had a test done. The test came back 100% no dna matched. She is 3 months old. How do I go about getting my name off the birth certificate. What forms or where do I have to go. I live in Oregon

    I would get an attorney. You will have to file a petition in Family court to remove yourself as the father. If you want to file yourself, contact the court for what you have to file.
