The test came back 100% no dna matched. She is 3 months old. How do I go about getting my name off the birth certificate. What forms or where do I have to go. I live in Oregon

I would get an attorney. You will have to file a petition in Family court to remove yourself as the father. If you want to file yourself, contact the court for what you have to file.
