    Request for modification for Child Support
    I live out of state and would like to request a modification in the dollar amount of
    My child support payments due to my changed financial situation. The original amount
    Was ordered from New York Family Court.

    Would anyone know the form that I would request for this modification? I cannot
    Afford to travel back and forth to New York due to finances so can I request this
    Modification through the mail or by telephone?

    You will have to get the address of the court unless you want to give the name of the city/county in NY

    You would have to contact the court in New York where the original order was issued and find out what you would need to file the motion for modification of your support order. And you will have to appear at the hearing in person ; that's inevitable. You might or might not be able to submit the actual paperwork by mail as opposed to in person ; again, check with the court clerk. And be prepared to pay some sort of filing fee. You also may have to personally serve a copy of your motion to your adversary, i.e. the custodial parent who receives your support payments.

    AND WAIVER OF PERSONAL APPEARANCE

    It was in the modification stuff so I thought he could do that.

    I didn't actually look at the links so that might be an option as well. I'm sure that eventually technology will make its way into the justice system (though not without a lot of controversy I'm sure.)
