Hi,

Last year, when my J-2 was about to expire I applied for visa extension.
Three weeks after that I received new DS 2019 (that is valid tile 2011) but I didn't get a new visa stamp in my passport.

My questions:

1. What is the visa expiration date for me right now?
2. What date should I put into filed 6c of W-7 form?
3. Should it be the date from my DS2019 (may 2011) or the original date that was on my vista stamp (February 2009).
4. Do I have attache the DS2019 to my W-7?
