    First its not a good idea to piggyback your question on someone else's. This can lead to confusion. You should start a new thread. So I've moved your question to its own thread.

    Second, didn't you bother to read the thread you piggybacked on?

    Do you have someone ready to adopt the child? If not, then its unlikley a court will allow him to sign his rights over.

    If your husband is willing to adopt the child, and the biofather is willing to relinquish his parental rights, then contact an attorney. They will prepare all the paperwork needed.

    If none of the above applies... then there is no use for the forms. A father cannot relinquish his parental rights unless there is someone to adopt the child in his place. And if you have someone to adopt the child, the biofather needs to WILLINGLY relinquish his rights to allow for the adoption.
