    What do I have to do to add my name to my son's birth certificate?
    Also, he was born in Colorado and we currently live in Massachusetts.

    Are you living with the child's mother ? Have you established paternity in any method, such as DNA tests ? Is the child's mother OK with adding you to the birth certificate

    Both parents would need to sign and submit this form. If the other parent will not agree, you should contact the Colorado Department of Public Health and Environment.

    I do live with the mother and she is OK with adding me to the birth certificate. Also, right now he has a hyphenated name... {Names removed}". Will that form you told me to fill out change that as well?
