As you may know, there are 50 states in the U.S. Child support is determined by the laws of one of those states (or a territory, if not within a state); it is not a Federal question. So the details may differ from state to state.
But generally, the custodial parent (mother, in this case), would file a lawsuit in court, asking for custody and child support. Each state has a formula for support, factoring in such viariables as the income of each parent, etc. If both parents are in agreement, this can be done by stipulation.
Child support is sometimes administratively ordered by state agencies established for that purpose.
But again, the details will depend on which state the mother and child are in.
