    Does employer have to offer same package to all employees
    Can different packages be offered to each employee or same package for everyone? Pertaining to how much employer pays and how much employee pays.

    What percentage is required by employer and what percentage by employee.

    As in my companies, each 'group' can have different packages of insurance; management, employees, etc. However within each group the benefits must be the same.

    There are no set percentages. Its totally up to the company.

    Packages can be different for different classes of employee, but its rare that you can offer separate packages for individuals. And you definitely can't discriminate against an individual in offering a package.
