Harbor breeze motor is not working after 5 years of service

AND, Breaker, switch, wire? Little more help please.

What is the fan doing? Is it not turning?

How do you know it's the motor?

I have a Harbor Breeze fan with 3 lights on it in my son's bed room. The fan seems like it only has one speed. I tried all 3 speeds, but the fan only has a slow to med speed. Also the fan has a little whine sound to it. I have 3 other Harbor Breeze fans in my house and all 3 speeds work on them and no whine sound. I would love to have some help on this problem.
