    Correcting my 17 year old grandson's birth certificate by adding his deceased father
    My grandson is 17 and shortly after his birth, a dna test was done to determine paterinity of his father. His biological father agreed to the test and acknowledged his paternity. On the birth certificate the father is left blank. His father died several years ago. He did receive SSI checks from the government on behalf of his father.

    My grandson woul like the birth certificate to be corrected with his father's name in the correct place.

    How do we go about changing this?

    Contact your state's (or country's) bureau of vital records for the process. You will need a copy of the acknowledgement of paternity and/or DNA test results.

    Also take a copy of the check reciept with you from the SSI statement. That will help bolster your claim too.
