    How do I apply for a loan from my 401K?
    I need to borrow 1500 from my 401k to purchase a refrigerater to pay back over 6 - 12 months.

    You contact whomever holds the 401K for you, typically a Mutual Fund Company.

    Usually this is done through your benefits department, not directly with the investment administrator. Your benefits department should have the forms. Not all 401(k) plans allow for loans. Have you contacted them?
