I need to evict my 32 year old daughter who is disrespectful to both of us and do not follow the house rules. She does not pay us rent even though she works full time. Where can I download a form to file for a formal eviction order? Do I need to see a lawyer for this?

Most often you can download them off the internet from your local government. Since you didn't mention where you are we can't tell you.

Have you given her written notice yet? That is the first step. Then you file in court. The court papers should be able to be gotten from you local court house or online with your local government. I don't know where you are but where I am there is a fee that has to be paid when you file with to get the court date.

First, I moved this from Family Law, since it does not matter that she is a relative. She is considered a resident and has all the rights of a tenant even though she doesn't pay rent or have a lease.

Second, any question on law needs to include your general locale as laws vary by area. Without knowing where you are, we can't tell you the exact procedure.

But generally, the procedure goes like this:

1) Give her a written notice to vacate, usually within 30 days, but can vary by area. This can be very simple:
To <insert name>
This is notice to vacate the premises at <insert address> by <insert date>. Failure to vacate by the deadline will result in the pursuit of an eviction order through the courts.
2) If she does not vacate by the deadline go to your local housing court and apply for an eviction order, you can get the forms from the court.
3) A hearing will be scheduled where she can show cause why an eviction order shouldn't be issued. Unless she can convince the court, they will issue an order to vacate with a deadline a few days in the future.
4) if she still doesn't vacate you hire a sheriff to physically remove her.
