    When will I receive my w-2 form from Louisiana Unemployment for 2012?

    First, go to the Louisiana Department of Labor website; you might be able to download directly from there.

    If not, get the phone number to call and ask when it will be mailed to you.

    Actually, for unemployment, it IS a Form 1099-G.
