Hi.
How are you?
I have a 3 years old child and I think he is far behind from other child's in terms of speaking,listening and understanding. He says many words. Count from 1to20. When listen some sound then react . Like if a icecream person come and ring his bell then he immediately goes to him and ask for ice cream but not with words. He goes to wash room when ask.
But very few things he understand in this age.
Please suggest what I need to do??

You need to have him evaluated. I'd start with his Pediatrician who will recommend testing.

He may hear the ice cream bell but not hear well enough to learn words.
Is there any way locally where his hearing can be tested?
There are little tests you can do, such as letting him play with a toy with someone else, and leave the room, then walk back in 2 minutes from behind him where he can't see or feel your feet through the floor, or hear you enter, and say his name in a low but normal tone. Try that with short questions too, until you are sure he can or cannot hear what other people can hear.
Aside from hearing it gets more complicated, because to test for intelligence and psychological conditions requires professional testing, and even then mistakes can be made. Are you located near a university that might have such testing?

Thanks for your reply.
I went to children hospital and doctor said that there is nothing wrong with him. If he can hear you and can reply and speak a few words also then he will be OK.

He do hear things , suppose if I will say 1 then he will say 2 . If I will say 5 , he will say . And he counts from where I start. Like if I will say 10 then he will 11 and end in 20. If I say papa, he repeats with me papa. But then when I say some sentences then he hardly understand.

Suppose if I will say to him to go to washroom then he will go. Means he understand that. But if I will say sit down, he just looking at me shockinly and when I points that like me site down then he sits.

I'm not totally convinced that his hearing is good enough to hear words strung together. I say this as a woman in my 60s who is losing hearing in one ear, and I just can't 'hear' rapid speech all the time when it's coming from that one side, at least not enough to really understand it all.
But if he really can hear, maybe he needs a playgroup of toddlers to socialize with a few hours a day. Especially if you can't get expensive behavioral testing done. There may be nothing at all wrong with him. Some of the smartest, most accomplished people start with all sorts of delays like this.
