    How do I get my name on my child's Birth Certificate
    Never married to mother of my daughter. At time of birth I was not notified. My name was not given to my daughter or put on the Birth Certificate. This was not my choice. I want her to have my name and be on the certificate. I have DNA proof that I am the father. I live in Abq NM so does my daughter and her mother.

    You probably can not get her name changed without the mother's consent. Even had you been notified and included on the birth certificate, she had the right to name the child whatever she wanted.

    How did you obtain this DNA proof? Have you gone to court to be declared the legal father and be given some level of custody and visitation?
