    First, its not a good idea to piggyback your question on another thread. To avoid confusion, I've moved your question to its own thread.

    If taxes are not being withheld, there may be a form, but that would be up to YOUR payroll dept. we couldn't answer. For federal and state withholding there is the W4 form which indicates how many exemptions you want to claim. This is turn dictates the percent of withholding.
