    HIPAA violation?
    Dear Barrister,

    Are data collection tools such as treatment logs that hospitals generate on patients available to the patient it pertains to upon the patient's request under HIPAA laws?

    What sort of breach has occurred when unauthorized alterations to treatment logs are made by hospital employees?

    What is the magnitude of the above described breach?

    What section of HIPAA or other hospital policy does the breach violate?

    None of that is covered by HIPAA.

    It is not covered, since it appears the info is not being given out to others improperly.
    Also as to hospital policy, each hospital will and do have their own policy, which differs from hospital to hospital.
