    Go to the employer first. They are required to maintain their copies of W-2s for seven years.

    Next, contact the IRS and ask them to mail you a copy of your 2004 tax script. The W-2 information is contained in that tax script.

    If you need state tax information (will NOT be in the IRS tax script), you would need to contact the state tax authority.
