    How do put my name on a birth certificate?
    When I was born my parents didn't no what to name me so they never put a name on it after they finally named me

    It says female and my last name that's it when I was born they didn't no what to name me so I went home with no name and when they finale decided on a name they never remailed the birth certificate back in

    If you are in the US you can contact the Bureau of Vital Statistics in the state in which you were born.
