    Name change
    How can I change my child's name? When the biological father intentionally put false information on the birth certificate including his name (so my child has a stranger's last name) I was young and oblivious. I recently got informed he got deported and wants to correct his wrong doings by giving up his rights over to me and also accepting the name change. My question (or questions) is, how can I approach this to the courts? Should I just fill out a name change form with a notarized letter from his behave? Or will an informal name change application is what I should fill out?

    I would suggest consulting an attorney. Since he was deported, you may be able to have his rights terminated. Changing the birth certificate as part of that will get it all done at one time.
