This seems to be the only Forcible Detainer form KY uses.

I don't know if we may modify it , or how to correctly do so to reflect an eviction to terminate a month to month tenancy "without cause". We just want to clean it up and sell it now that my husband is retiring.

I have two questions.

1. Would we just "N/A" #2 and #3 on the form and write "without cause" in the space at #4?

2. Tenant has been in the house for five years. We first informed her of our plans 14 months ago, and have reminded several times since. But she is difficult, and has made no effort to find a new place. I expect her to claim it will be a hardship to move and will want months of extra time. I'm expecting to hear it will be psychologically , physically and financially stressful. The average wage in her profession is 50K +, but tenant was often late with $600 rent. Is she likely to be accommodated? I've never seen an eviction hearing, and have no idea what to expect.
