Can I get his rights revoked completely?
My daughter is going to be 3 yrs old in June,when I was pregnant with her my ex decided to rob a house about 2-3 months into my pregnancy knowing he would be caught and being he was on parolle he would do some time.he spent duration of my pregnancy incarcerated and up until our daughter was 4 months old he came home and didn't ever help with her.I left him when my daughter turned 6 months because he put his hands on me got very aggressive loud and nasty.he would ask to see her very rarely when he did he would have a bunch of friends with him and something always came up so he'd bring her back early that only lasted about 2 month after we split.he than got incarcerated again! He lost custody of his now 5 yr old son.his brother has full custody.my ex(daughters father) is a huge druggy he's violent and was recently arrested again for assaulting a 74 yr old man and is in jail with multiple felony charges.my boyfriend of 2 years has been raising my daughter with me and my daughter has no idea who her biological father is he never asks to see her since he took me to court for visitation and lied to the judge about why he wasn't present he told them he was in hospital with his mother when I told them he was incarcerated in county jail they laughed and dismissed the case.how hard would it be for me to have my daughters last name changed as well as his rights revoked?

I don't see grounds to have his rights revoked. You can certainly have visitation supervised.
How to get your daughter's last name changed depends on the State. You will need his consent.
Marry your boyfriend and see if the Court will allow him to adopt your daughter, again, if the father consents.
Was he always a violent druggie? We see this all the time. Sexual relationships, a child, the boyfriend is a druggie and a convict. You didn't notice this when you were sleeping with him?
Another child born into a bad situation. Three years old with Daddy and The Boyfriend. Sad.

Only a court can terminate parental rights and courts are very reluctant to do so. Generally they will only do so to clear the way for an adoption or if the parent is a danger to the child.
So marry your boyfriend and apply for an adoption. Otherwise its unlikely you will get the name change without the father's consent.

A long prison sentence is a reason to terminate rights in some states, so you need an attorney review all the issues.
As for as his abuse to you, how many police reports are there, What evidence, photos and the such. Since unless you have proof, it did not happen.

He was clean when I got with him on parolle had gone to rehab and was doing good than I got pregnant and things went down hill.like I said for amonth or so he asked to see her once in awhile than it stopped he started lying doing drugs turned into a criminal.courts awarded his brother and his wife full custody of his son.I have full custody of our daughter.he does not pay for anything for either child obviously because he spends most of his time behind bars or he's out dealing drugs or whatever and never has a legit job.we had supervised visitation but he seen her once for an hour than tried getting them unsupervised that's when the courts threw the case because he was locked up.I don't want my daughter to have my bfs last name I would like her to take my name.she was never born into a bad situation because she's NEVER been exposed to his life she never seen him alone without someone I knew being around him(his sister or my family) and my boyfriend she calls daddy who I am now engaged to

You cannot change her name without her father's consent.
The fact that he chooses not to work doesn't mean that you shouldn't get a support order. Maybe some day, some time, some place, he will find employment.
Of course, that's a big maybe.
