    There are a number of ways to do this, but since the problem gives you a point and the slope, why not first write the equation in point-slope form? Then, all you have to do is some trivial algebra to solve the equation for y, and then it will automatically be of the form y=mx+b (i.e. slope-intercept form).

    Remember that point-slope form is



    where and are the x- and y-coordinates of any point on the line.
