    Can a mother add her last name to her child's existing name?
    I gave my daughter her father's last name on the birth certificate ( although his name is NOT on her birth certificate), I want to add my last name to it. I live in New York. Please advise and help.

    Yes all you need to do is get the fathers permission

    You will probably need to go to court to effect the change. This may require the father's approval.
