    Defective balancer where the rubber sandwich has deteriorated between the inner and outer halves.

    In any case, get it replaced ASAP. Problems that can result are as varied as Crankshaft breaking to cracks in the main bearing webs in the block.

    Improper torque, deteriorated rubber (age), not using thread locker (if required).
