    Ackerj01 - I'm going to take a wild guess here and assume that you meant form 1099R. The 1099R is issued if you receive income from pensions, annuities, retirement or profit sharing plans - including 401(k)'s - IRAs, etc. Box 7 shows the "Distribution Code," which provides info as to whether an early withdrawal penalty may be due. If you dd indeed mean form 1099-R, please tell us what code is showing in Box 7 and we can interpret it for you.
