Call the HR department of your old employer and ask whether you have a 401(k) with them. They can tell you who the plan administrator is (it's typically a brokerage house, bank, or insurance company) and how to contact them for more info. Be sure to give your updated address to both your old employer and the 401(k) plan administrator so that they don't lose track of you again. I would also recommend that once you find this account, think about rolling it to an IRA, so that you have control of it and don't forget about it.
