    I am 14 years old need to apply for passport but my dad disappeared
    I am 14 years old and want to apply for passport but my disappeared long time ago, couldn't locate his where about. INS need my dad signature.

    A US passport? Your guardian (probably your mother) can go to Court and get the Court's permission for her to sign if you can prove that you cannot locate your father.

    Is there a record somewhere - family court, for example - that he has vanished, that he doesn't pay support, that your mother has tried to locate him?
