    First, contact your employer and ask them to mail you a copy (the IRS and state tax authorities WILL take photocopies of the W-2).

    If that is not feasible, then contact the IRS and ask for a tax script for 2004 and 2005. There is no charge for this script, which should have the W-2 information. Once you have the script, any competent tax professional can reproduce a W-2 for you using tax software. You will have to also ask the state for their version of the tax script, because the IRS tax script does not normally have the state tax withholding.
