Extension to be filed for both fed and state?
    Hi
Let me know if I have to file separate extension requests for both federal and state returns if I intend to file returns only after 15-Apr-10. (extending to meet SPT)

Also is there a form or template available for requesting extension.

Yes - you need to file separate extension request forms for federal and state. The federal form is 4868 - more info here: Extension of Time to File Your Tax Return

Note that the extension gives you more time to complete the tax return paperwork, but if you owe any taxes they must still be paid by April 15.
