    What documents should be attached with the W7 form for ITIN application?
    Hi

    I am on an H4 visa.
    I want to apply for ITIN number along with my husbands's federal tax return.

    I have filled the W7 form.

    1) What documents are required to support the W7 form?

    2) Also I changed the last name from maiden to married on my passport but it is still the same on my visa.
    Will that be a problem when I apply for ITIN?

    3) Lastly how long does it take to get the ITIN from IRS?

    Hi there I need to send back my 7w form back to USA however they are asking my to also send my passport ,I am unable to do this as using passport can I send anything in stead i.e. driving licence also have you a telly phone number for this office not on forms many thanks Nina
