Well, you probably have a very gummed up throttle body, and egr passages, and idle air control valve.(IAC).
You need to replace the ECT sensor. That's the engine coolant temp sensor which controls you fuel injection volume.
Passenger seat removal is required to get at it.

It will cost you about 75.00. You must lift the passenger seat out, and bracket for the seat, door sill plate, lift carpet, there are 12 (12mm bolts) that hold the floor pan down under the pass carpet.
The ECT sensor is forward of the valve cover sitting on top, and has a green connector going to it. You can't miss it.
It will greatly improve warm up performance, but you sound to have other issues going on also.
I highly recommend changing out the spark plug wires while you're in there if needed. It's your call/money. GET TOYOTA PARTS!

Remember the ole' saying. Garbage in, garbage out. If the computer sees garbage inputs, then it runs like garbage.
Computers NEED to "see" these sensors. If they don't, the old Previa will fall flat on it's face!
