What happens after the first apparence in a violation of probation case and why can't the person be released today?

The person is "usually" given the sentence on the first visit. At least this is true in my area. There is no second appearance.

The person is not simply let "go" because he/she didn't meet the terms of probation and can be forced to serve the original sentence if probation had not been granted.

Now there is a second crime on top of the first one.

I was a probation officer in Maryland for seven years and there is really not enough information given in the question to provide an adequate answer. At that hearing the judge makes a determination as to whether the person is going to be released pending the violation of probation hearing and, if so, what the conditions of release will be. Or you may be describing a case in which a person went into court for a violation of probation hearing and was not released. In that case the person got jail time for violating the probation.
