Can I change my name and social security number? If so, how do I go about doing so?

You must have a very good reason to do this, and it reads like you want to disappear.

You would have to go through the courts to change your name and you certainly would have to have a good reason to do this, as far as the courts are concerned.

You can't change your social security number. The only time it might ever be changed is if a mistake was made and a duplicate issued.

Why would you want to, by the way?

As to a name change, you would need to go to your local family court and petition for a change and you will need to give a reason.

A name change is easy, you go to court house and pick a name, pay a fee and do it.
