+1. You must make a nonimmigrant visa appointment at a US consulate or embassy, typically, your home consulate, unless you've have the visa issued to you before. Appointments can be made online, generally, and you will need to complete the DS-156 form and possibly others. There will be a visa fee and possibly a "visa reciprocity" fee. You will need your original approval notice, a certified copy of the petition, your original educational documents, passports, etc. If you are married or have children, they can apply at the same time for the H-4 (bring marriage certificate and birth certificates).
