1) If you arrived in the U.S. in 2003 or earlier, then you were a resident alien for 2008. That being the case, your employer correctly withheld the Social Security (and probably Medicare) taxes from your salary while on OPT. You are NOT entitled to a refund.
2) Negative; your are a resident alien. However, you CAN claim the $5,450 standard deduction.
3) Yes.
4) Probably Form 1040, though you may be able to file Form 1040A or 1040EZ.
