Finding an exact match for even a 5 year old tile can be almost impossible. Start carrying a piece of it and visit as many tile show rooms as you can. Ask for help, don't just walk the aisles by yourself. If the makers name is on the tile a saleman may be able to find info on the net to see if the tile is still made or warehoused. I'd keep my fingers crossed.
