    Etiquette in different last names
There is a married couple, but she did not take his last name. When formally addressing something, should it be addressed to Mr. & Mrs. (his last name), or Mr. (his last name) & Ms. (her last name), or another way?

As you know that she does not use his name, the proper thing to do is to address them individually as per your second suggestion : Mr. (his last name) & Ms. (her last name).

While my wife "officially" added my name after hers (like jane brown smith), many of her coworkers and friends still address her by her maiden name only... it was just easier for her professionally to keep the same name and I didn't mind one bit.

One way people have addressed this, in addition to the above post's recommendation (which I favor), is by addressing an envelope as: Smith Brown Family, for ex.

And then watching for return letters for how they seem to sign their cards and letters on the envelope.
