Normally you will have to make the request though your probation officer.
Each state, and perhaps even each court will have its own forms. But your first stop will be talking to the PO

Hello other:
There ain't no form.
However, before you make your request, I suggest you read the rules so that you can quote from the particular section and paragraph (if there is one) that provides for early termination. Ask your PO for a copy.
Then just send a certified letter making your request to the judge with a copy to your PO and to the prosecutor's office.

Wrong-in Florida you or your lawyer does it-i'm just looking for a request for an appearance for I can modify it

In Florida a Probation Officer can do this for you. A friend had her PO fill out the form and submit it to the judge on her behalf. The judge granted the early termination of probation for her. You must have completed a minimum of more than half of the time and paid any fines or fees in full at the time of the request as well. They must have changed the rules since I lived in Florida.

My husband is on probation (5 total, 2 of those under supervision & the rest "administrative"). Our PO mentioned that 6 months after the supervised part we would can ask for an early termination. Is that right? If it is, is it a lengthy process? Thanks!
