I worked for a medical practice in medical records and the physician would require us to alter records to prevent any legal action against him. What avenue can I pursue to turn him in for this?

Did you take before and after pictures of the docs? Do you have other proof? Will any of his present employees back you up? Has he been sued, and did he use a forged document in his defense??

If the above answers are NO, then you've got an uphill battle. You could certainly make a complaint with your states medical board, but without proof, you're opening yourself up to be sued by HIM.

So go GET your proof, then go GIT HIM!!
