I have a question on Form 8233 Exemption from withholding on compensation for independent (and certain dependent) personal services of a nonresident alien individual. I am filling out the form for an Italian professor who provided a 3-day consulting service for an industry in the US in January and got paid for his services (or should get paid if we manage to fill in the form). I have resolved the US-Italy tax treaty but I can't find the following information:

Line 6. US Visa type. I think being a business trip it should be B-1. The Visa gets taken away from the passport when you leave the US. But this should be correct, shouldn't it?

Line 9a and 9b. Current nonimmigrant status and date your current nonimmigrant status expires. The accounting department for which the Italian professor provided the services told us to look at form I-94, which is the form they make you fill out at the airport, I think, but he doesn't have this form anymore, because you have to give it in again at the airport. Where can I find this information or what should the answers be?
