    Completing form 843
    I am filling out the form 843 claim for refund and request for abatement.
    In line 5 b dates of payments , what should I write here ?
    Line 6 : indicate the type of return filed to which the tax, interest, penalty,.
    Do they mean the original form which is in my case 1040 ? Or what ?
    In line 7 : explain... and show the computation of the amount shown on line 2
    I don't understand that ? Could any one verify this to me please ?

    Line #5b: Improperly-withheld FICA taxes for 1Jan - 31 Dec?

    Line #6: Form 941.

    For Line #7, you have to show how much FICA (Social Security and Medicare) taxes you need to be refunded to you.
