    You need to go to a good accountant firm and pay them a couple of hunderd dollars you would be supries what they can do for you they would get a biger retrun

    For early withdrawal form 401K, your plan administrator will send you a 1099-R form, showing income received and taxes withheld. You show the income on line 16 of your 1040, and the 10% penalty on line 60. If you are under 59-1/2 years of age, the withdrawal is subject to the 10% penalty.
