    Does a quadro need be notarized
    Is a quadro valid if downloaded from internet and does it have to be notarized

    I have read where even with attorneys, it can be difficult at times to meet all of the requirements.

    Yes the form can be downloaded, just be sure to use one approved for your state.( where court action is)

    Also in the state I am looking at, I see no requirement for notary, (remember that is just a witness of signing) but to be honest, I would get it, just to be sure, why waste time filing, if they turn it down for that later.

    More then likely it doesn't need a notary but it will require proper service and be prepared for a joinder from the party administering the retirement plan.

    Notarization simply means that a licensed party confirmed the identity of the signer. A QDRO is submitted by one spouse against the other spouse's retirement benefits. I'm not sure if both spouses need to sign. If so, then it would be a good idea to notarize the signature of the one who's retirement benefits are being attached. But I don't think that's the way it works.
